Breathing is the only part of the autonomic system you can operate directly, which is why it is the most reliable regulation tool available and also the most oversold. The useful mechanism is narrow: slow the rate, make the exhale longer than the inhale, breathe through the nose. That produces a measurable shift within a minute or two. Almost everything else marketed as breathwork — the fast, intense, cathartic styles — does something quite different, and a few of those practices carry real risk.
Why the exhale is the whole mechanism
Heart rate is not constant. It rises slightly during inhalation and falls during exhalation — respiratory sinus arrhythmia, and the reason the out-breath is where the leverage sits. Vagal outflow to the heart is comparatively suppressed on the in-breath and restored on the out-breath, so lengthening exhalation biases the balance parasympathetic. That is the physiology behind every calming breath technique, whatever it is called.
Two practical consequences follow, and they cover most of what is worth knowing.
Around six breaths a minute is where the effect peaks. Slow breathing at roughly this rate produces the largest oscillations in heart rate and the strongest reported effects on HRV — Zaccaro and colleagues’ systematic review of slow breathing (Frontiers in Human Neuroscience, 2018) found consistent increases in HRV and reports of reduced arousal, while noting the studies are small and varied. Six a minute is a four-second inhale and a six-second exhale, which is why that ratio keeps appearing.
Nose, not mouth. Nasal breathing warms and humidifies the air, produces nitric oxide, and naturally slows the rate. Mouth breathing tends to be faster and shallower. This is one of the few claims in the field with an uncomplicated physiological basis.
Four techniques worth knowing
Everything below is free, needs no app, and works sitting in a car.
1. The physiological sigh — fastest for acute stress. Two inhales through the nose, the second a shorter top-up on top of the first, then a long slow exhale through the mouth. Repeat one to three times. It reinflates collapsed alveoli and offloads carbon dioxide efficiently, and a randomised comparison by Balban and colleagues (Cell Reports Medicine, 2023) found five minutes daily of cyclic sighing improved mood and reduced respiratory rate more than mindfulness meditation or two other breathing patterns. It is the one I use most, because it works in under thirty seconds and is invisible to anyone watching.
2. Extended exhale (4–6 or 4–8) — the general-purpose one. In through the nose for four, out through the nose for six or eight. Five minutes. No counting once the rhythm settles. This is the plain version of the six-breaths-a-minute finding and the default worth building a habit around.
3. Box breathing (4–4–4–4) — for focus rather than sedation. Equal inhale, hold, exhale, hold. It steadies and concentrates rather than settling you down, since the ratio does not favour the exhale. Useful before something demanding; less useful at midnight.
4. Resonance breathing — if you want to be precise. Everyone has a rate, usually between 4.5 and 6.5 breaths a minute, at which heart rate oscillations are largest. Finding yours requires a bit of trial and error or an HRV device. The gains over plain 4–6 are marginal for most people, and it matters mainly if you are training deliberately.
Note what is missing from that list: nothing requires twenty minutes, a course, or a facilitator. If you only keep one, keep the physiological sigh.
How it differs from vagus exercises and from general regulation
These three terms are used interchangeably and are not the same thing.
Breathwork is one input, operating through respiratory control of autonomic balance. It acts within seconds and its effects are short-lived unless repeated.
Vagus nerve exercises — humming, gargling, cold exposure, the ear and neck techniques — are a mixed bag with weaker evidence, and the causal story is looser than the marketing implies. Slow exhalation is the best-supported item usually filed under that heading, which is to say it is mostly this page under a different name. Nervous system regulation is the whole picture: sleep, food, light, movement, load, relationships, and how much genuine recovery time exists in a week. Breathing is a lever within it and cannot compensate for the rest. Someone sleeping five hours in an unmanageable situation will not breathe their way out, which is the point made at length in the main regulation guide.
The kind of breathwork to be careful with
This section matters more than the rest of the page, because the risks are real and rarely mentioned in the classes selling them.
Fast or forceful styles — holotropic, rebirthing, Wim Hof rounds, kapalabhati, bhastrika, and most workshop breathwork — are hyperventilatory by design. Blowing off carbon dioxide lowers blood CO2, which constricts cerebral blood vessels and raises blood pH. The tingling, the hand cramps, the lightheadedness and the emotional intensity are that chemistry, not a release of stored trauma. Fainting is a documented consequence, and the widely reported cases of drowning during breath-hold practice in water are the reason no version of this belongs anywhere near a pool or bath.
Avoid fast or breath-hold practices if you:
- are pregnant
- have epilepsy or a seizure history
- have uncontrolled high blood pressure, a heart condition or an aneurysm history
- have glaucoma or raised intraocular pressure
- have a history of panic attacks — hyperventilation reliably reproduces panic sensations
- have psychosis or bipolar disorder in your history
- are in or near water, or driving
None of that applies to slow breathing with a long exhale, which is safe for almost everyone. The distinction is simply slow versus fast, and it is worth holding onto because both are sold under the same word.
What the evidence doesn’t support
- That intense breathwork releases trauma stored in the body. The physical sensations are the documented effects of low CO2. Emotion can surface, and the proposed mechanism is not established.
- That breathing resets the nervous system. It shifts state for minutes to hours. Baseline change requires repetition over weeks, and mostly requires reducing the load.
- That more is better. Longer and more forceful sessions are not more regulating; past a point they are activating.
- That specific counts are magic. 4–7–8, 4–6, 5–5 — what matters is a slow rate and an exhale longer than the inhale. No trial establishes a superior number.
- That it treats anxiety disorders or PTSD. It can help in the moment and alongside treatment; it is not a substitute for care with actual outcome trials behind it.
- That alkalising the blood detoxifies anything. It does not; it briefly changes pH and cerebral blood flow, which is a different claim entirely.

Does breathwork regulate the nervous system?
